When Counseling Weight Loss, It’s Style That Makes A Difference

Most doctors are spending a good deal of time counseling their patients about diet and weight loss, but for the most part, it isn’t making any difference, according to a new study appearing in the American Journal of Preventive Medicine.Weight Loss  Difference

Duke University Medical Center researchers recorded the conversations between 40 primary care physicians and 461 of their overweight or obese patients over an 18-month period. Investigators didn’t tell them what they were listening for – only that they wanted to record the encounters to see how doctors talked about health. What they were really tracking, however, was how much time was spent and, specifically, how doctors talked with their patients about diet, exercise and weight loss.

Turns out, doctors talk about weight quite a bit. Physicians discussed weight with patients in 69 percent of the encounters.

“We found that on average, physicians spent about three and a half minutes talking about diet and weight loss,” says Kathryn Pollak, PhD, a member of the Cancer Prevention Program in the Duke Comprehensive Cancer Center and the lead author of the study. “That may not sound like much, but it amounts to about 15 percent of the time of the average office visit, which ran about 20 minutes. So the good news is, physicians realize how important the issue is, and they are making a point to talk about it.”Weight Loss Difference

But the data showed that there was no difference in weight loss between those patients who received counseling and those who did not get it.

Investigators found a very different story, however, when they dug a little deeper and divided the doctors and their patients into groups according to communication styles.

They found that three months after the office visit, patients whose doctors talked about diet and weight loss in a more motivational fashion – using predominantly reflective or empathic statements — were much more likely to lose weight, compared to those whose physicians used a more judgmental or confrontational style of communication. Patients whose physicians communicated well lost about 3.5 pounds three months after the visit, which is substantial given most overweight and obese patients gain weight over time, says Pollak.

“Patients don’t like to be told what to do, and they are generally not going to question or talk back to their doctor,” says Pollak. “But what happens when doctors use reflective statements or a more motivational and empathic approach, it changes the relationship; the patient becomes more of an equal, more of a partner in care.”

“So, for example, instead of asking a question like ‘So, you can’t fit exercise into your day?’ a physician might say something like ‘It sounds like you’re finding it hard to find time to exercise.’ That kind of reflection seems to help patients open up more and give more meaningful information to doctors.”

Just over a third of the physicians in the study reported having some training in behavioral counseling, and Pollak says such instruction is becoming a higher priority item in traditional medical education.

She says the study is the largest of its kind and the first to examine not only the frequency of diet and weight counseling in physician office visits, but also the quality of the counseling and its impact on patients.

“Results of the study indicate that physicians may indeed have the power to help patients change their eating and exercise habits,” says Pollak.

She says future research should examine whether weight loss that resulted from the sessions can be maintained over time, and whether improving physicians’ training in behavioral communications would lead to more effective counseling and even greater weight loss.

Scientists Find Natural Weight Loss Secret – Clinical Trial Confirms Simple No-Diet Appetite Control Method Really Works

If something sounds too good to be true, the old saying goes, it probably is. But here’s an exception: scientists have documented that an all natural appetite-control agent with no side effects really does help people shed excess weight — and what’s more, it’s practically free. Scientists just reported the results of a new Scientists Find Natural Weight Loss Secret – Clinical Trialclinical trial of this weight loss-promoting liquid at the 240th National Meeting of the American Chemical Society (ACS) in Boston.

So what’s in this “magic elixir”? Plain water.

“We are presenting results of the first randomized controlled intervention trial demonstrating that increased water consumption is an effective weight loss strategy,” scientist Brenda Davy, Ph.D., of Virginia Tech in Blacksburg, Virginia, said in a statement to the media. “We found in earlier studies that middle aged and older people who drank two cups of water right before eating a meal ate between 75 and 90 fewer calories during that meal. In this recent study, we found that over the course of 12 weeks, dieters who drank water before meals, three times per day, lost about 5 pounds more than dieters who did not increase their water intake.”

Scientists Find Natural Weight Loss SecretDr. Davy, who was the senior author of the study, explained that folklore and anecdotal experiences have long suggested that drinking more water might promote weight loss. In  addition, previous research has suggested that drinking water before meals reduces the intake of calories. But until now, there has never the scientific “gold standard” evidence from a randomized, controlled clinical trial comparing weight loss among dieters who drank water before meals to dieters who did not drink the extra water.

The new study involved 48 adults between the ages of 55 and 75 years. They were divided into two groups and, although all research subjects ate a low-calorie diet for the study, one group drank 2 cups of water before meals and the other group did not. After 12 weeks, the water drinkers had lost about 15.5 pounds, while the non-water drinkers lost about 11 pounds.

So why is drinking water before a meal an effective way to lose weight? Dr. Davy said it may simply fill up the stomach with something that has no calories. And when people feel full, they usually eat fewer calorie-containing food during their meal. In addition, Dr. Davy added, drinking water may also help people lose weight if they drink plain water instead of calorie-laden sweet beverages, such as sodas.

CSleep Loss Limits Fat Lossutting back on sleep reduces the benefits of dieting, according to a study in the Annals of Internal Medicine.

When dieters in the study got a full night’s sleep, they lost the same amount of weight as when they slept less. When dieters got adequate sleep, however, more than half of the weight they lost was fat. When they cut back on their sleep, only one-fourth of their weight loss came from fat.

They also felt hungrier. When sleep was restricted, dieters produced higher levels of ghrelin, a hormone that triggers hunger and reduces energy expenditure.

“If your goal is to lose fat, skipping sleep is like poking sticks in your bicycle wheels,” said study director Plamen Penev, MD, PhD, assistant professor of medicine at the University of Chicago. “Cutting back on sleep, a behavior that is ubiquitous in modern society, appears to compromise efforts to lose fat through dieting. In our study it reduced fat loss by 55 percent.”

The study, performed at the University of Chicago’s General Clinical Resource Center, followed 10 overweight but healthy volunteers aged 35 to 49 with a body mass index ranging from 25, considered overweight, to 32, considered obese. Participants were placed on an individualized, balanced diet, with calories restricted to 90 percent of what each person needed to maintain his or her weight without exercise.

Each participant was studied twice: once for 14 days in the laboratory with an 8.5-hour period set aside for sleep, and once for 14 days with only 5.5 hours for sleep. They spent their waking hours engaged in home- or office-like work or leisure activities.

During the two-week, 8.5-hours-in-bed phase, volunteers slept an average of 7 hours and 25 minutes each night. In the 5.5-hour phase, they slept 5 hours and 14 minutes, or more than two hours less. The number of calories they consumed, about 1,450 per day, was kept the same.

The volunteers lost an average of 6.6 pounds during each 14-day session. During weeks with adequate sleep, they lost 3.1 pounds of fat and 3.3 pounds of fat-free body mass, mostly protein. During the short-sleep weeks, participants lost an average of 1.3 pounds of fat and 5.3 pounds of fat-free mass.

Getting adequate sleep also helped control the dieters’ hunger. Average levels of ghrelin did not change when dieters spent 8.5 hours in bed. When they spent 5.5 hours in bed, their ghrelin levels rose over two weeks from 75 ng/L to 84 ng/L.

Higher ghrelin levels have been shown to “reduce energy expenditure, stimulate hunger and food intake, promote retention of fat, and increase hepatic glucose production to support the availability of fuel to glucose dependent tissues,” the authors note. “In our experiment, sleep restriction was accompanied by a similar pattern of increased hunger and … reduced oxidation of fat.”

The tightly controlled circumstances of this study may actually have masked some of sleep’s benefits for dieters, suggested Penev. Study subjects did not have access to extra calories. This may have helped dieters to “stick with their lower-calorie meal plans despite increased hunger in the presence of sleep restriction,” he said.

The message for people trying to lose weight is clear, Penev said. “For the first time, we have evidence that the amount of sleep makes a big difference on the results of dietary interventions. One should not ignore the way they sleep when going on a diet. Obtaining adequate sleep may enhance the beneficial effects of a diet. Not getting enough sleep could defeat the desired effects.”

Resveratrol is a type of phytonutrient known as a polyphenol. Found in the skin of grapes, wine, grape juice, peanuts, and berries, it has often been hailed as a life-extending natural compound. After all, research in mice and lab rats Resveratrolhas indicated it can protect those animals from obesity and diabetes and has anti-cancer, anti-inflammatory and blood-sugar-lowering effects, too. However, rats and mice are rodents — and their physiology is in many ways different from the primate family that includes apes, monkeys and, most importantly, human beings.

But now for the first time a study has shown resveratrol has the ability to rev up metabolism and spark weight loss in primates — and that means the polyphenol might have weight loss and even anti-aging and life-extending benefits in people, too.

Fabienne Aujard, from the Centre National de la Recherche Scientifique in Paris, France, worked with a team of scientists to document how a diet supplemented with resveratrol impacted the weight, metabolism and energy intake of six mouse lemurs. (Despite their names, mouse lemurs have nothing to do with rodents. Found only on the African island nation of Madagascar, Weight Loss Studythey are mouse-sized primates — the group that includes apes and humans.)

The study, which was just published in the BMC Physiology journal, showed that after four weeks of resveratrol supplementation there was a significant decrease in the animals’ food intake along with a reduction in the body-mass gain lemurs normally experience in winter. The response to the resveratrol supplementation also involved significant changes in the animals’ body temperatures. The researchers noted that resveratrol appears to reduce weight by increasing satiety (the feeling of being full) and also by increasing the resting metabolic rate (the amount of energy expended while at rest) — so the animals burned up more calories even when not exercising.

“We’ve found that lemurs eating a diet supplemented with the compound (resveratrol) decreased their energy intake by 13 percent and increased their resting metabolic rate by 29 percent,” Dr. Aujard said in a statement to the press. “These results provide novel information on the potential effects of resveratrol on energy metabolism and control of body mass in a primate. The physiological benefits of resveratrol are currently under intensive investigation, with recent work suggesting that it could be a good candidate for the development of obesity therapies.”

Weight Loss : Eat To Live – Not Live To Eat

Do you find yourself eating food when you’re not even hungry? I’ve noticed that others, myself included, often eat food without even stopping to assess whether we’re hungry or not. I encourage you to get in touch with your body and eat only when you need to. Not Live To EatThis entails being aware of your body’s needs and not eating food if you’re not hungry.

When I speak to people who maintain a healthy weight level without any real effort, I find one of the keys for them is they only eat when they’re hungry. If they’re not hungry they don’t eat. This group of people generally find it hard to understand how the rest of us can eat food if we’re not hungry.

As most of you know, when we eat food when we’re not hungry, this is usually being triggered by something else which is going on in our lives. One of the first steps to identifying these triggers and dealing with them is for you to be aware of the times when you’re hungry and the times when you’re not.

This will take some time and effort to start with, but as with any habit, it becomes easier over time. Eventually, you will unconsciously go through the process of determining when you’re really hungry.

First, you need to start thinking consciously about your hunger levels. I would suggest that in your mind or on paper you create a ladder with 10 rungs and label each rung with a level of hunger, according to what feels right for you.

 Not Live To EatFor instance, rung 1 might be “I’ve just eaten and feel full up.” Rung 2 might be “I ate an hour ago and still feel satisfied.” Rung 5 might be “I’m starting to feel slightly hungry.” Rung 7 might be “I’m feeling hungry now and it would be a good time for me to eat.” Rung 10 might be “I’m absolutely starving and want to devour anything.”

The wording of these rungs needs to suit you and reflect yourself. I suggest that round about rung 6 or 7 is when you really start to feel hungry and therefore it’s appropriate to eat. Rung 1 should be not needing to eat at all and rung 10 needs to be the other extreme, where you’re far too hungry.

When you want to eat food, take a look at this ladder and determine for yourself where you are on it. If you’re on a low rung, perhaps your urge to eat is being triggered by something else and it would be beneficial to take a look at this.

I would suggest the best time to eat food is round about rung 6 or 7 when you’re genuinely hungry and need food more than anything else. I would also encourage you not to get to rung 9 or 10 before you eat, or the hunger you’re feeling will overwhelm your good sense and you’re likely to overeat in order to feel satisfied.

Weight Loss Drugs – Produce Only Minimal Weight Loss, Even After Taking Them for Years

Weight loss drugs may result only in minor weight loss, even after long-term use, according to a new study conducted by Brazilian and Canadian researchers and publishWeigh Loss Drugsed in the British Medical Journal.

Researchers conducted meta-analyses of a number of studies conducted on the weight-loss drugs orlistat (marketed as Xenical and Alli), rimonabant (marketed as Acomplia) and sibutramine (marketed as Meridia), and found that users lost an average of less than 11 pounds, even after one to four years of use. Several key indicators of cardiovascular health were improved by taking the drugs, however.

Researchers examined 16 studies on orlistat, which operates by preventing the body from digesting fats. The average long-term user of orlistat lost only 7 pounds and had reduced diabetes risk, blood pressuWeight Lossre and cholesterol. As many as 30 percent of users experienced digestive side effects.

Ten tests on sibutramine were also examined, along with four on rimonabant. Both drugs work by interrupting neural signals in the brain.

Sibutramine was found to reduce patients’ weight by an average of only 9 pounds. In up to 20 percent of patients, however, it induced side effects including insomnia, nausea, and elevated blood pressure and pulse.

Rimonabant users lost an average of 11 pounds. Six percent of users experienced an elevated rate of mood disorders, however.

After receiving reports of psychiatric side effects such as anxiety and depression, the FDA refused to approve rimonabant for U.S. sale last year. Orlistat, in contrast, is approved for over-the-counter sale in a weakened form (Alli).

The move to sell weight loss drugs over the counter has drawn substantial criticism, including in an editorial accompanying the recent study.

“Selling anti-obesity drugs over the counter will perpetuate the myth that obesity can be fixed simply by popping a pill,” Dr. Gareth Williams of the University of Bristol wrote.
